Informed Consent Form for In-Person Services
Due to the continuing public health concerns, you are asked to complete an in-person therapy consent form before we can meet face-to-face.

Scheduling
My online scheduling system, located here, will allow you to select whether your appointment will be virtual or in-person.
(Please make a note of which option you have chosen for which date, as I may be holding virtual sessions off-site that day.)
If we schedule an appointment at the end of a session, or over the phone/email, please specify if you want to meet in-person or virtually. Otherwise, I will assume you prefer the same format as the last session.
Parking
The Highland Building is just off Ponce de Leon Ave NE. There is free, plentiful street parking on Seminole Ave NE and Blue Ridge Ave NE surrounding the building. (Note that Blue Ridge is a one-way street!)
There is also a public parking lot directly across Seminole from the building, for a small fee. The small parking lot attached to the building is reserved for other building tenants — please don’t park there as you may be towed.
Arrival
The building can be accessed through the front door facing Seminole. It is secured with a key code; I will send you with the code via email before the first in-person therapy session.
There is a callbox, but please be aware that I may not always be available to buzz you in when you arrive — for instance, if I am speaking with another client. For that reason, I encourage you to keep the building key code somewhere accessible for when you need it, such as a notes app on your mobile device.
You are required by building management to wear a mask at all times while in the building.
The office is located on third floor. As you exit the elevator, turn right and you will find Suite 310 around the corner toward the end of the hallway.
In-person, face-to-face sessions are being scheduled with enough time between sessions for me to thoroughly sanitize the therapy room and waiting room. Please do not enter the waiting room prior to 5 minutes before the start time of your appointment.
Restrooms
So that you may plan comfortably, please be aware that the building has single-occupant restroom facilities on every floor. There are gender-specified restrooms just down the hall from my office on the third floor, and there are also all-gender restroom facilities on the other side of the elevator.
Health and Safety
All areas in both the therapy room and waiting room that may have been touched by me or another person prior to your session will be wiped down with disposable antiviral wipes.
Please wear a mask at all times while in the waiting room and therapy room, and maintain a physical distance of 6 feet from any other people.
Hand sanitizer has been made available for your and my use in multiple locations throughout both the waiting room and the therapy room.
Coffee, Tea, Water, and More
Please make yourself comfortable in the waiting room. There is a water cooler available for your use that dispenses cold, hot, and lukewarm water. The hot water can be used to make hot tea or instant coffee which are available at the refreshment station.
There is also a small refrigerator with bottled water, sparkling water, and sodas that you may help yourself to.
